Birth Plan Template — Preferences Without False Promises
A useful birth plan is a one-page set of preferences covering communication, comfort, monitoring, pain relief, support, immediate newborn care and what matters if plans change.
Reviewed and updated:
The one-page structure
- Who you want with you
- How you prefer information and consent conversations
- Movement, positions and environment
- Pain-relief preferences and options you want explained
- Immediate skin-to-skin and feeding preferences
- Preferences if assisted birth or caesarean becomes recommended
Write priorities, not a perfect script
Birth can change quickly. “Please explain options and give us a moment to ask questions when safe” is often more useful than trying to predict every intervention.
Review it before the final weeks
Take the plan to an antenatal appointment. Ask which choices depend on your hospital, medical circumstances or local policy. Give your partner a copy and store another with your notes.
Common questions
Can the hospital ignore my birth plan?
A plan records preferences, but urgent medical needs can change what is safest. Your team should still explain recommendations and involve you in decisions whenever possible.
How long should a birth plan be?
One page is ideal for quick use. Put the most important preferences first.
